Water Billing Agreement 
Print this out, write your name under tenant at the top and fill in your social security number under tenant social security number. In the middle of the form you need to put the inside and outside meter reading along with the date that you read the meter (your move in date), print your name at the bottom, sign and date with your phone number under the tenant section. Once you have filled this out fax it to 614-467-2091.
